3. Affiant is the sun�iving spouse of Joe Bender (hereafter "Decedenr`), �aho died a resident of
Gibson County, Indiana, on or about August 16, 2008.
4. At the time of death, Decedent was the owner with Affiant, as tenants by the entirety, of the
following described real estate located in Gibson County, Indiana:
See Exhibit A attached hereto and made a part hereof.

5. Decedent and Affiant were husband and wife continuously from and afrer September 2, 19>0
through the date of death of Decedent.
6. The property taY code numbers for the Real Estate are as follows: (See Exhibit A)
7. Title to Real Estate vested in Affiant by operation of law at the death of Decedent,
and the Auditor of Gibson County, Indiana; is requested to show transfer of Real Estate to Affiant on the
transfer records of the Auditor's Office in said County pursuant to this Affidavit.
8. Henceforth, a property tax statement for Real Estate should be sent to Affiant at the followino
