1. The above-named decedent died testate on the 25`" day ofJanuary, 2002, �i-hile domiciled
in Lee Counry; State of Florida. being sixty-seven (67 ) years of age.
2. I was personally well acquainted with William R. Brucls, the decedent, family and near
relatives of the decedent, durine the decedent's lifetime, the affiant having knowv the decedeni for
more than � � vears..
3. The decedent was married at the time of his death.
4. The decedent did leave a�vill.
5. There �ti�as no administration on the estate of the decedent in Indiana.
6. No petition for the appointment of a personal representative of said decedent is pendine
in any Court in this State and that forty-five (4�) days have elapsed since the death of the decedent.
7. There aze no children of any deceased son or daughter of the decedent.
8. No child or children were adopted by the decedent and the decedent did not have any
stepchildren.

less, bein� a part of the same real estate described in Deed Record No. 94
Page 335, Office of the Recorder of Gibson County, Indiana.
Tract 2: A part of the North half of fractional section twenty-ttvo (22),
to�vnship one (1) south, range ten (10) �vest, bounded and described as
follo�vs, to-�vit: Begin one hundred thirty three and three tenths (133.3) rods
east of the northwest corner of the north�vest quarter of the said fractional

acres, more or less.
SUBJECT TO all legal highways, rights-of-�vay, restrictions, conditions,
covenants, leases, easements, reservations of oil, gas, coal, minerals and
mineral rights and limitations of record, and also to applicable zoning laws,
if any.
13. There are no creditors for the estate.
13. Jolene C. Brucks, Carol�n Lee Ann Consolato and Jane Michelle Cole are entitled to
the real estate as the sole sun�iving heirs of William R. Brucks.
14. The eross value of the estate of the decedent, William R. Brucks, as deternuned for the
purposes of Federal Estate tares, was less than the value required for the filing of a Federal Estate
Tar Retum. As a consequence thereof the decedent s estate was not subject to Federal Estate Tax.
