AFFIDAVIT FOR TRANSFER OF REAL ESTATE ON AUDITOR'S RECORDS
Anne C. Smith and Andre�v C. �'�'elbom, being first dul�� swom upon their respective oaths, state:
Thev are competent adults.
Each has personal kno���ledee of the matters stated herein.
3. Alice M. Cah�ert died on April �; 2008. At the lime of her death, she was residing at the
Transcendent Health Care facilit�� located near O���ensville. Indiana, but her domicile was still Posev Countl,
Indiana.
4. That at the time of her death, Alice M. Calvert o�ti�ned a life estate in the follo«�ing described real
estate located in Gibson Counri�. Indiana:
Thirt�� (30) acres off the south end of the east part of the Southwest quarter of Section 27; in
to«mship 3 south, ranee 12 west, and described as follo���s: Commencine at the southeast corner
of said south�vest quarter of said section; thence north 60 rods and 9 links, more or less, to a
stone in the half section line; thence ���est 80 rods: thence south to the section line: thence east to
the place of beeinnine.
(hereafter "Real Estate")
�. That the life estate o�med in the Real Estate b�� Alice M. Cal��ert at the time of her death ��•as
inherited bv Alice M. CalveR from Mildred R. Williams under the terms of the Last Will and Testament of
Mildred R. Williams. Mildred R. Williams died Februarv 28. 1970. Because the life estate of Alice M. Calvert
�+�as inherited by her and not reserved bv her, the extineuishment of her life estate by her death does not
constitute a taxable transfer respecting the Real Estate for Indiana inheritance tax purposes or federal estate tax
purposes.
6. That the undersiened affiants, Anne C. Smith and Andre�v C. l'�'elbom, were, immediately prior
to the death of Alice M. Cah�ert, [he o«mers of the remainder interest in the Real Estate subject to the life estate
of Alice M. Cabert, and that upon the death of Alice M. Cah�ert, her life estate was estineuished; and Andre�v
C. Welbom and Anne C. Smith are no�i� the o���ners, as equal [enants in common, of fee simple tide to the Real
Estate.

7. A copy of the death certificate for Alice M. Calvert is attached hereto, made a part hereof, and
labeled Exhibit A.
8. That the propert�- tax code for the real estate is as follo�as: 26-17-27-300-000.232-021.
9. Henceforth; a property tax statement for an undivided one-half (1/2) interest in the Real Estate
should be sent to Anne C. Smith at 214 East Fourth Street, Huntinebure, R�! 47542, and a separate property taz
statemen[ for the other undivided one-half (1/2) interest in the Real Estate should be sent to Andre�i� C. Welbom

10. That the undersiened hereby requests that the Gibson County Auditor sho��� the extinguishment
of the life estate of Alice M. Cah�ert on the records of the Office of the Auditor of Gibson Countv. Indiana. and
