AFFIDAVIT OF SURVIVORSHIP

JO ELLEN DAVIS, being first duly sworn upon her oath,
deposes and says:
1. That she make this affidavit for the purpose of

having the following described real'estate situateiin Gibson
County, State of Indiana,transferred to her name individually,
which property is described as.follows, to-wit_: _. _
Lot number 91 in Tower Heights, a sub-division of a
part of the Northwest quarter of section 12, township 2
south, range 11 west, as said'lot is designated on the
recorded plat of said sub-division.
2. Affiant says further that the above described
real estate was heretofore listed in the Auditor's Office of
Gibson County, Indiana, under the name of Don W.Davis and Jo Ellen
Davis, husband and wife.
3. Affiant says further that said above described property
was owned by the said Don w, Davis and Jo Ellen Davis, husband and
wife, as Tenants by the Entireties.
4. Affiant says further that Don w. Davis departed this
life testate on the 29th day of January, 1973, leaving a Last Will
and Testament that was duly admitted to probate in the Gibson Circuit
Court, Gibson County, Indiana on the 7th day of March, 1973, and this
affiant was the sole and only beneficiary under said will.
5. This affidavit is made for the purpose of transferring
the above described real estate to the name of Jo Ellen Davis,
individually.
