SUMMARY DISSOLUTION OF MARRIAGE DECREE
(January 2, 2003)
Comes now the Petitioner, Paula Jane Garrett, in person and by counsel, Steven
L. Whitehead, and comes now the Respondent, Troy Dale Garrett, in person and by
counsel, Susan R. Todino, and the parties submit their Decree of Dissolution and Mediated
Settlement Agreement to the Court for consideration as follows:
PENDENCY OF ACTION: That this Petition has been filed and has been pending
in this Court for more than sixty (60) days and was properly initiated by service of
Summons on the Husband under Trial Rule Four (4).
RESIDENCY: That the requirement of residence in Gibson County for more than
three (3) months and the State of Indiana for more than six (6) months by at least one (1)
of the parties before the filing of the herein cause has been met.
JURISDICTION: That the recitations as to Parties, Pendency of Action and
Residency, as stated above, are true and the Court has jurisdiction of this matter.
PREGNANCY: That the Wife is not pregnant.
REQUEST FOR RELIEF: That an irretrievable breakdown has occurred in the
herein marriage and said marriage should be dissolved.
SETTLEMENT AGREEMENT: That the settlement agreement submitted to the
Court for the Court's consideration and approval during the proceedings, relating to the
issues of the marriage, was made by and between the parties subsequent to their

incurred in connection with such enforcement proceedings.
FULL UNDERSTANDING.
26. Each party fully understands all of the terms of this agreement, and the terms
represent and constitute the entire understanding between them. Each party has read this
document and finds it to be in accordance with his or her understanding, and each
voluntarily executes this document and affixes his or her signature.
RELEASE.
27. Each party does hereby release and discharge the other from all other
claims, rights and duties arising out of said marital relationship, and said parties mutually
agree that each parry here�o may fr�ely s�ll, e�cumber or othez��ise dispose of his o� her
own property by gift, bill of sale or a last will and testament, except as specifically set out
herein. Each parry is by these presents, hereby barred from any and all rights or claims
by way of inheritance, descent and distribution, all rights and claims as widow, widower,
heir, distribute, survivor or next of kin, and all other rights or claims whatsoever in or to the
estate of the other, whether real or personal, and whether now owned or hereinafter
required which may, in any manner, arise or occur by virtue of said marriage.
BINDING OF HEIRS.
28. All the covenants, promises, stipulations, agreements and provisions herein
contained shall bind the heirs, executors, administrators, personal representatives and
assigns of the parties hereto.
SEVERABILITY AND ENFORCE ABILITY.
29. If any provision of this agreement is heid to be unenforceable, all other
provisions shall nevertheless continue in full force and effect.
FORMER OR MAIDEN NAME.
30. That the Wife's former name be restored to Paula Jane Holzmeyer.
IT IS THEREFORE CONSIDERED, ORDERED, ADJUDGED AND DECREED by
the Court:
DISSOLUTION OF MARRIAGE: That the bonds of matrimony heretofore existing
between the parties herein be and they hereby are dissolved; that the marriage between
