1. That your affiant; Louise T. Jenkins, and Dennis Wayne Jenkins were mamed on June 16.
1999, which marital relationship continued unbroken from the date of the marriage until the death
of said Dennis Wayne Jenkins; on December 31, 2008.

No. 20050000631 S; said Louise T. Jenkins and Dennis Wayne Jenkins received title as tenants by
