AFFIDAVIT OF SURVIVORSHIP
The undersigned, LOUISE TRAYLOR JE\'KI\'S; being first duly swom upon her oath,
deposes and says as follows:
1. That your affiant, Louise Traylor Jenkins, and Dennis Wayne Jenkins were married on
June 16, 1999, which marital relationship continued unbroken from the date of the mamage until
the death of said Dennis �Vayne Jenkins, on December 31; 2003.
2. That pursuant to a �Varranty Deed dated December 7; 2004, and recorded as Instrument
No. 200400005�09, said Louise Traylor Jenkins and Dennis Wayne Jenkins received title as tenants
by the entireties, to the following described real estate in Gibson County; Indiana, to-wit:
A part of the South part of the Southeast Quarter of the Southeast Quarter of Section
Thirty-Four (34), Township Two (2) South, RanQe Nine (9) West, in Columbia
Township, Gibson Counry; Indiana; and further described as follows:
The point of beginnine may be found by measuring from an iron sucker rod set and
marking the Southeast Comer of said Section Thirty-Four (34), along the East line
of said Section, a true bearing of North 06 De�'ees �7 Minute East, a distance of Six
Hundred Tweniy and No Tenths (620.0) Feet to a point; thence by measuring parallel
to the South line of said Section, NoRh 38 Degrees and 30 Minutes West, a distance
of Seven Hundred Seventeen and Thirty-Nine Hundredths (71739) Feet to a five-
eights (�/8) inch iron set; thence bear South 09 Degrees 43 Minutes East, a distance
of Sisty-Eight and \'ineteen Hundredths (68.19) Feet to a five-eights (�/8) inch iron
set mazking the point of beginning; thence continue South 09 Degrees.48 Minutes
East, a distance of Two Hundred and \'o Hundredths (200.00) Feet to a five-eights
(�/S) inch iron set in the North right of way of a Fifty and No Hundredths (50.00)
foot wide proposed road; thence beQin by bearing I�'orth S 1 Degrees and 34 Minutes
East and along the curve to the right having a radius of Six Hundred Fifty-Three and
Sixty-Seven Hundredths (6�3.67) Feet a distance of Two Hundred Sixty-Six and
Fifty-Nine Hundredths (266.59) Feet to a fi��e-eiehts (5/8) inch iron; thence bear
North 02 Degrees �S Vlinutes East, a distance of Two Hundred Forty-0ne and
Ninety-Four Hundredths (241.94) Feet to a five-eights (5/8) inch iron set thence

The above tract is commonly referred to as Robert F. Traylor Tract Two (2), as
surve}'ed on October 31, 1977 by Lowell E. Morrison; Surveyor-Engineer; under
Indiana Land Surveyor No. 101 S4.

SUBIECT TO all legal hiehways; rights-of-way; restrictions, conditions, covenants,
leases, easements and limitations of record, and aiso to applicable zoning laws, if

3. That by reason oFthe death of said Dennis Wayne Jenkins on December 31; 2008; your
affiant, Louise Traylor Jenl:ins, as surviving tenant by the entireties; became the sole owner in fee
simple of the above described tract of real estate.
4. -['hat to your affiant's l;nowledge; all legal debts and claims against the decedent or his
estate have been paid in full.
�. Your affiant, Louise Traylor Jenkins, further certifies that the estate of Dennis Wayne
Jenl:ins is not taxable for Federal Estate or Indiana Inheritance Tax purposes due to the fact that all
assets of said estate were jointly held with or passed ro your affiant, the surviving spouse of said
decedent.
6. That this Affidavit is made for the purpose of transfemng the above described real estate
to the name of LOUISE TRAYLOR JENKNS as the surviving owner thereof on the records of
Gibson County, Indiana.
