AFFIDAVIT FOR TRANSFER OF
REAL ESTATE ON AUDITOR'S RECbRDS
Patricia J. Smith, being first duly sworn upon her oath,
deposes and says:
l. That she is a competent adult.
2. That she—has personal knowledge of the matters stated
herein.
3. That she is the surviving spouse of John J. Smith who
died a resident of Gibson County, Indiana, on or about the 12th
day of August, 1988.
4. That there were no proceedings for the administration o�
an estate for John J. Smith and none are contemplated; and that
no liability for Indiana inheritance tax or federal estate tax
was created at the death of John J. Smith.
5. That at the time of the death of John J. Smith, he was
the owner with this affiant, as tenants by the entirety, of the
following described real estate in Gibson County, Indiana, to-
wit:
A part of Lot Eleven (11) in Brenton's Addition to the
town of Fort Branch, bounded as follows: Beginning at
a point 50 feet north of the southwest corner of said
Lot 11, and run thence north 75 feet; thence east to
the east line thereof; thence south 75 feet; thence
west to the place of beginning.
(hereinafter the "real estate").
6. That John J. Smith and this affiant were husband and
wife continuously from and after February 27, 1962 through the
date of death of John J. Smith.
7. That title to the real estate vested in this affiant by
operation of law at the death of John J. Smith, and the Auditor
of Gibson County, Indiana, is requested to show transfer of said
real estate to this affiant on the transfer records of the
Auditor's Office in said County pursuant to this Affidavit.
