Comes now Jessie Anvine Tooley, Petitioner herein, and files his verified Petition for the
adoption of Lisa Marie (Ludwick) Dages, which Petition is on file with the Court and a part of
the Court's record.
And said Petitioner appearing in person and by his attomey, Steven L. Whitehead, and
submiu his verified Petition to this Court for hearing and decree on the same, and the Court
having heazd the evidence thereon and being duly advised in the premises finds that the
allegations of the Petition are true; that the requirements of a reportive investigation is deemed
inappropriate wiih regazd to this proceeding; that all necessary consents have been duly signed,
acknowledged and filed with this Court and aze a part of this Court's record; that the adoption
prayed for is desired by all parties concemed, at,d that said prayer should be granted and that the
costs of this action have been paid in full.
IT IS TI-�REFORE CONSIDERED, OF:DERED, ADNDGED AND DECREED by the
Court that the above entitled Petition for the adoption of Lisa Marie (Ludwick) Dages be and the
same is hereby approved and ganted, and that s�id Lisa Marie (Ludwick) Dages is hereby
declazed to be the daughter and heir at law of Jessie Anvine Tooley, and that Lisa Marie
(Ludwick) Dages desires that her name be changed after said adoption to that of Lisa Marie
Tooley.
