1. That Karen L. Teel is a competent adult.
2. That Kareu L. Teel has personal knowledge of the matters stated herein.
3. That Karen L. Teel is the surviving spouse of Jonnie J. Teel, who died a resident of Gibson County,
Indiana, on or about the 2"d day of July, 200�.
4. That at the time of the death of Jonnie J. Teel, Jonnie J. Teel was the owner with this affiant, as
