AFFIDAVIT FOR TRANSFER OF
REAL ESTATE ON AUDITOR'S RECORDS
Mary Ellen Ziliak, being first duly sworn upon her oath,
deposes and says:
1. That she is a competent adult.
2. That she has personal knowledqe of the matters stated
herein.
3. That she is the surviving spouse of Leonard I. Ziliak,
who died a resident of Gibson County, Indiana, on or about the
13th day of May, 1989.
4. That at the time of the death of Leonard I. Ziliak,
Leonard I. Ziliak was the owner with this affiant, as tenants by
the entirety, of the following described real estate in Gibson
County, Indiana, to-wit:
(See Exhibit A attached hereto and
made a part hereof by reference.)
(hereinafter the "real estate").
5. That Leonard I. Ziliak and this affiant were husband and
wife continuously from and after , 19.Sd through
the date of death of Leonard I. Zilial.
6. That title to the real estate vested in this affiant by
operation of law at the death of Leonard I. Ziliak, and the
Auditor of Gibson County, Indiana, is requested to show transfer
of said real estate to this affiant on the transfer records of
the Auditor's Office in said County pursuant to this Affidavit.
