AFFIDAVIT FOR TRANSFER OF REAL PROPERTY
1. That the above-named decedent died testate on the 12'h day of October, 2012, while
domiciled in Gibson County, Indiana.
2. That forty-five (45) days have elapsed since the death of the decedent.
3. That no application or petition for the appointment of a personal representative is pending
or has been granted in any jurisdiction nor is any administration contemplated.
4. It appears that the decedent's gross probate estate, less liens and encumbrances, does not .
exceed the sum of the following: fifty thousand dollars ($ 0,000.00), the costs and expenses of
administration, and reasonable funeral expenses.
5. That among the decedent's probate assets is a parcel of real estate which was owned by the
decedent located in Gibson County, Indiana, more particularly described as follows:

A part of Lot 28 in the Original Plan of the Town of Francisco, bounded as follows:
Begin in the west line of said lot at a point 18 feet from the northwest corner thereof
and run thence southwesterly 132 feet to the southwest corner thereof; thence
southeasterly 100 feet to the southeast corner thereof; thence northeasterly along the
east line of said lot 132 feet to a point 18 feet from the northeast corner thereof; thence
northwesterly 100 feet to the place of beginning.
EXCEPTING THEREFROM, A part of Lot #2S in the Original Plan of the Town of N Francisco, Indiana and bounded as follows: Begin at a 5/S inch iron found in the ig

6. That the individuals entitled to an equal share in the real estate as a result of the decedent's •
death are the decedent's devisees as provided according to the Will, spread of record on

Valerie Jean Thomas, Adult Daughter
7. Michael A. Thomas and Valerie Jean Thomas hereby relinquish their interest in the real
estate and request that the real estate be transferred TCHaroi J. Thomas.
8. That the gross value of the estate of the decedent, Myrtle Lucille Thomas, as determined
for the purposes of Federal Estate taxes, was less than the value required for the filing of a Federal
Estate Tax Return. As a consequence thereof, the decedent's estate was not subject to Federal Estate
Tax.
9. That the decedent's estate was not subject to Indiana Inheritance Tax.
