t1FFIDAVIT OF DEATH AND HEIRSHIP
OF BLt1NCHE TOOLEY . DECEASED
1. The above-named decedent died intestate on the 23rd day of Apri1,1996, �vhile
domiciled in Gibson County, Indiana, being Eighty-five (85) years of age.
2. I �vas personally �vell acquainted �vith Blanche Tooley, the decedent, family and
near relatives of the decedent, during the decedenYs lifetime, the affiant having known
the decedent all m�� life.
3. The decedent �vas married at the time of her death.
4. The decedent did not leave a wiil.
5. There �vas no administration on the estate of the decedent.
6. No petition for the appointment of a personal representative of said decedent
is pending in any Court in this State and that forty-five (45) days have elapsed since the
death of the decedent.
7. There are no children of any deceased son or daughter of the decedent.
8. No child or children �vere adopted bp the decedent and the decedent did not
have any stepchildren.
9. The follo�ving named persons are the only surviving heirs of the decedent:
Benjamin F. Tooley, husband
Jerry R. Tooley, son
Jeanetta Finch, daughter.
10. The value of the decedenYs o oss probate estate, less liens and encumbrances,
does not exceed the sum of the allot�=ance provided by I.C. 29-1-4-1, the costs and
expenses oi administration and reasonable funeral expenses.
11. Among the decedenYs probate assets is a parcel of real estate tvhich �vas
owned by the decedent in her name only located in Gibson County, Indiana, more

particularly described as follo�vs:
The south half of the north�vest quarter of section Thirty-t�vo (32), township one
(1) south, range nine (9) west, containing eighty (80) acres, except sixty (60) acres
in the form of a parallelogram off the north side thereof.
Commonly kno�vn as 650 E, Francisco, IN 47649.
SUBJECT TO all legal high�vays, rights-of-�vay, restrictions, conditions, covenants,
leases, easements and limitations of record, and also to applicable zoning la�vs if
any.
12. There are no creditors for the estate. `
13. Benjamin F. Tooley, Jerry R. Tooley and Jeanetta Finch are entitled to the real
estate as the sole surviving heir(s) of Blanche Tooley.
14. The a oss value of the estate of the decedent, Blanche Tooley, as determined
for the purposes of Federal Estate taxes, �vas less than the value required for the filing of
a Federal Estate Tax Return. As a consequence thereof, the decedenYs estate �vas not
subject to Federal Estate Tax.
15. The decedent's estate �vas not subject to Indiana Inheritance Tax.
